Does my insurance plan cover my visit/surgery with your office?

Midwest Eye Care attempts to contract will all major health insurance carriers in the region for medical and surgical care. We accept assignment for Medicare, Medicaid, Blue Cross Blue Shield, United HealthCare, Humana, Coventry Nebraska, and Midlands Choice, among others. If we do not participate with your insurance company, you may be subject to a higher co-pay or deductible when your insurance company processes your claim. Please be sure to ask if you’re unsure whether we participate with your insurance company. If you don’t provide us with accurate insurance information at least several days prior to your appointment, it will be is nearly impossible for our office to warn you if your insurance company will provide reduced benefits at your visit. If you do provide accurate insurance information in a timely manner, we’ll do our best to confirm your eligibility and benefits.

Several of our doctors participate with Vision Service Plan (VSP) and EyeMed, and a participating provider is available for exams at all five of our metro locations. We do not participate with Spectera (United Healthcare’s routine plan) or Davis Vision. However, we do provide discounts to patients of the following insurers who wish to utilize our services outside their formal vision plan:

  • Blue Cross Blue Shield of Nebraska (17.5% off optical charges. Disposable contact lenses are excluded.)
  • United HealthCare (20% off optical charges. Disposable contact lenses are excluded.)

In some instances, patients have the option of paying an additional insurance premium to their employer for routine eye care and eye wear coverage. Many routine vision plans offer only a limited number of frame and/or lens options under their standard coverage, so the cost of the additional premiums and upgrade costs may outweigh the benefits of the vision plan.
